Nayantara was spotted, while she was still in college, by director Sathyan Anthikkad, who cast her opposite Jayaram in the 2003 Malayalam film Manassinakkare. Later, she co-starred alongside Mohanlal in Natturajavu and in Fazil's Vismayathumbathu. She was also cast with Mammootty in Thaskaraveeran and Raapakal.

She subsequently started appearing in Tamil and Telugu films. In 2005, she was cast opposite Sarath Kumar in Hari's Ayya, debuting in the Tamil film industry, and followed it up with Chandramukhi. The comedy horror film, in which she starred alongside Rajinikanth, became one of the most successful Tamil films in history, running for over 800 days in theatres,[7] eventually turning Nayantara into one of the most-sought after actresses in Tamil.[8] Later that year her third Tamil film, A. R. Murugadoss' Ghajini, in which she played a secondary female character, again was a commercially highly successful venture. However, she later disclosed that appearing Ghajini was a big mistake and the "worst decision" she ever took.[8] She then made her debut in Telugu, starring in the film Lakshmi, following which she performed in Boss, alongside Venkatesh and Nagarjuna respectively. She regained her star billing in Kollywood acting opposite Ajith Kumar in the Vishnuvardhan-directed Billa. The gangster film, a remake of the same-titled 1980 Tamil film, went on to become a blockbuster success at the box office,[9] while Nayantara received many accolades for her performance as Sasha

Shetty made her acting debut in Puri Jagannath's 2005 Telugu film Super alongside Akkineni Nagarjuna. The same year, she acted in another film Mahanandi, which was an unsuccessful venture. In 2006, she had four releases, the first being S. S. Rajamouli's Vikramarkudu opposite Raviteja, in which she portrayed the character Neeraja Goswami. The film went on to become very successful and got her much recognition, that eventually made her very popular in Andhra Pradesh.[4][5] She next had starred in the film Astram, a remake of the 1999 Hindi film Sarfarosh, following which she debuted in the Tamil film industry, acting in the Sundar C.-directed action flick Rendu, sharing screen space with R. Madhavan. Later that year, she made a special appearance in A. R. Murugadoss' directorial debut in Telugu, Stalin.

Her first release in 2007 was Lakshyam, after which she starred in Raghava Lawrence's Don opposite Nagarjuna again, of which particularly the former did very well at the box office.[6] 2008 she had appeared in six films overall; Okka Magaadu was the first release, in which she played one of three female leads. Her subsequent releases Swagatam alongside Jagapati Babu and Bhumika Chawla and Baladur opposite Ravi Teja again, garnered poor reviews and box office returns. She next appeared in the film Souryam pairing with Gopichand again, which became a box office hit.
